A watch can display an electrocardiogram-like graph in less than a minute, but it is not reading the heart in the same way a 12-lead clinical machine does. Understanding how the ECG in smartwatches works helps you leverage this feature without expecting diagnoses the device cannot provide. For daily use, it can be a useful signal to observe changes in heart rhythm and to converse with your doctor with more information.
How the ECG in smartwatches works
The ECG (electrocardiogram) function records the electrical activity that makes the heart contract. Each heartbeat begins with an electrical impulse. A compatible smartwatch detects small electrical differences in the skin and converts them into a graph that represents the heart's rhythm for a few seconds.
To achieve this, the watch needs contact at two points. One is usually on the back, resting on the wrist, and the other can be integrated into the crown, a button, or the metallic frame. By touching this second point with a finger from the opposite hand, you close a very small electrical circuit between both arms, allowing the device to take the measurement.
The reading usually lasts between 30 seconds and a minute. During this time, the watch analyzes the regularity of the intervals between beats. Afterward, the application can indicate that the rhythm appears regular, that there are signs consistent with irregular rhythm, or that the reading was inconclusive. The result is saved, depending on the model and configuration, for later review or to share during a medical consultation.
It does not produce shocks or an electrical sensation. The smartwatch only captures natural body signals through its electrodes.
It's a single-lead ECG, not a complete examination
This point marks an important difference. An ECG performed in a clinic typically uses multiple leads, with electrodes on the chest, arms, and legs. This allows observing the heart's electrical activity from different angles and detecting a wider variety of abnormalities.
A smartwatch ECG is usually single-lead. It is practical, quick, and convenient for a spot reading from home, work, or after noticing palpitations, but it offers a limited view. It can help identify rhythm patterns, especially some cases of atrial fibrillation, if the model has that validated function. It does not replace a medical study nor does it serve to rule out a heart attack, chest pain, or other emergencies.
ECG, heart rate, and oxygen: they are not the same
Many users see various health data on their watch and assume they are all measured the same way. In reality, each function uses different sensors and calculations.
The heart rate monitor that works continuously usually uses green light. This system is known as PPG or photoplethysmography: it illuminates the skin and calculates changes in blood flow with each pulse. It is very useful for seeing pulsations during a walk, a workout, or rest, but it does not directly measure the heart's electrical signal.
The ECG uses electrodes and requires you to initiate the reading, keep your arm still, and touch the indicated contact point. That's why it offers a different type of data: not just how many times your heart beats per minute, but whether the pattern between beats appears uniform or irregular.
Blood oxygen measurement, when available, also uses light with different wavelengths. And blood pressure functions present in some accessible watches should be interpreted with even more caution: they do not replace a cuff sphygmomanometer or professional evaluation. Comparing functions by name is not enough; it's advisable to check which sensor each model actually includes and how it performs the measurement.
What it can and cannot detect
A smartwatch with an ECG can be useful for recording a reading when you feel a racing heart, pauses, chest fluttering, or a rhythm that seems strange. It can also help maintain a history of episodes that don't occur precisely during a medical visit. This context can be valuable for your doctor.
However, a rhythm result with no signs of irregularity does not mean everything is fine. The watch may not detect episodes that come and go, rhythms outside the categories it analyzes, or structural heart problems. Nor does it measure pressure inside the arteries or confirm causes of fatigue, dizziness, or shortness of breath.
If you experience chest pain or pressure, difficulty breathing, fainting, sudden weakness, cold sweat, or pain extending to your arm, back, or jaw, do not wait to take an ECG with your watch. Call 911 or seek emergency care. In these situations, time and clinical evaluation matter more than a screen notification.
